How to Create Form

Thank you, after all i found this forum. after a long time of searching in the net.

i'll just want to ask from those who have and idea about this.

we're using an ERP program here which is the only way to modify the existing form is through VBA. and now we're able to add a New Form for as to input new data and save to our existing table..

the scenario goes this.. when we click the button, the new form should appear...
