Adding radio buttons and counter on excel test sheet?

I am working on a excel format which is actually a small test (pl see the attached doc). Every time I click Take test button, random questions appear from different chapters. Now i need to put radio button after every option & a Submit button at the end of the test. On the click of which a counter will record the score. Request you to help me work this out.
