Custom Email Forms

I am trying to create a custom email form that people can use but currently when I send it to people they view the default Outlook form. How can I get it so that they view it in the form that I sent?

Where are they saving it? If they click Open | Form, do they choose from the right option in the drop down box? Generally it's not in the default category.
