fill-in form

In word there is the very simple thing called the fill-in form.
I want to use this, but with a difference that i don't want the user to fill-in something in the popup, but i want him to see a couple of options in the pop-up (some sort of combobox).

Is there a way tot do this?

Any help is appreciated.
