How to show text in another Document .

Hello ,
Please help me following attached file .
I wanna show another document in Name,Address,Heading (only 3 lines) .Same 3 line many document in write and stores in one folder.When Macro
run , it is 3 line appear in another document. Example:

1.Name
(Address)
Heading

2.Name
(Address)
Heading
etc.............

Depending on how you have things set up, perhaps use an INCLUDETEXT field.
