how to create a custom form

I want to create a new form with 3 columns: KAE (name of people), percent, and third column is colour for percent column. The number of KAE change by time, that I can't add control at runtime. Any one can help me how to create this form? thanks very much

First I would create a query to return all the desired records. Next create a form in continuous view to show all the records.
