Hi All,
I am new for Pivot Table and I am stuck in this problem.
I have to create a report as in Sheet1 in attachment from Sheet 2 in attachment.
Please help!!

data in sheet2 may not be sufficient.
btw,
select a cell for pivot table.
ribbon, insert, pivot table.
select the data which will be summarized (sheet2 A1:C7)
from the window which appears on the right;
- drag and drop first Facility and then Affected Dept to "row labels"
- drag and drop Name to "column labels"
- drag and drop the column header the values of which will be summed, counted, averaged, etc to "values"
