Hi :help

I hope someone can help, I know nothing about macros but think that this is what I need to do this job, I have about a hundred individual forms that I need to get some data from and put into excel spreadsheet.

I have tried to attach a sample document so that you can see what word form looks like, I need the information that I have entered as TEST,DOB and if the box has been ticked to go onto excel with each injured person and details on there own row.

I hope someone can help with step by step instructions if possible, as I said I have never done this and imagine it might be complicated.
