Macro icons appearing multiple times

I have a Word 2010 template I created that contains several macros and each macro is assigned to a unique icon within the quick access toolbar. It was designed so that when a new document is created from the template the macros and associated quick access icons will appear so that users can run them. This is working fine however sometimes the icons appear multiple times in the quick access toolbar in the worksheet.

Does anyone know what could be causing this and how I can correct?
