UserForm Checkboxes

Greetings,

I am making a workbook for field engineers to use to track their activities.
I have created several UserForms that are pre-filled with what should be done for each given task.
These are checkbox based.
What I want to do is have whatever is checked be sent to the Daily Activities page (column E) when the user presses the button.

Since they could do multiple tasks over multiple days, the Daily Activities page is an ongoing entity. So whatever is being added from the UserForm should be added to the next available row.

I’m not used to using UserForms, so a little help would be appreciated.
