Word Form, Submit button ?

Ok, I made a simple Word Form, I would like to place a Submit button
at the buttom of the form, so that when the user finishes it, and pressed
the submit button, outlook will Open and it will attach that form in the email and prefill the to: to the email that I want to use:

Is this possible ?

I am talking about outlook, not outlook express.

Hi,
Can you zip and post a copy of your form with some sample data so we can see exactly the layout in order to determine how the fields etc. can be extracted.
