Solved: Form fields?

Hello everybody,

Okay, so I can't do Word - at all. I'm so horrible it's hard to describe how novice I am. Now I've seen some Word documents used as Forms, especially on the internet. I'd like to be able to make a form like this, but don't know how. What's the easiest way?

Here is an example Document.

Show the Forms toolbar and you can add Text Boxes or Check Boxes etc. Then when you have it setup the way you want protect the document for Forms.
