Solved: How do I create a "dialog box"...?

Hello
I want to create a 'dialog box' (if that's the right terminology for it)...basically a window/user interface with several fields, which the user fills in. Once they click the 'Enter' button, this data will then be entered into a spreadsheet as a new row.
Is dialog box the thing it is called? I tried typing this into the excel-help wizard thing but it didn't come up with what I wanted.

Can anyone point me in the right direction?
