Solved: Help on Concatenate 2 cells under one drop down????

When you open the template. There is a defect. Please ignore that.

I have already populated the test template:

1. Go to Main
2. Select Updates
3. Select Contact form, I want to display under Contact Name as:
Test20, Test2
Test30, Test3

This means I am displaying Last Name, First Name together from the form.

Each entry under Test1 shall have a corresponding Contact Name, which should be disabled after selecting Contact Name.

Right now, its only displaying Test 2 or Test 3 (which is first name) alone. And no Last Name.

I mean only on the Updates Form --> Contact , i want to display, "Last Name, First Name" under Contact Name.

Can anyone please help???
