    Help needed! VBA form

Hi, i really need some help here
i have an example vba form that want to use in another table that has more columns.
the only thing i need is to be able in the form to view all the columns and not only the 4 columns of the example
i attach here the example database and my table with the columns i need to add in the form

Can somebody help me or tell me how to do it?
    its really important for me!

    thanks

It isn't the same but it does everything yours does without code, and is therefore much simpler to extend.
